Seroprevalence and incidence of genital ulcer infections in a rural Ugandan population. (1/63)
OBJECTIVES: To determine age-sex specific seroprevalence and incidence rates of Treponema pallidum, Haemophilus ducreyi, and HSV-2; to assess the association between HIV-1 status and incidence of these STIs; and HSV-2 serostatus with number of lifetime sexual partners. METHODS: Antibodies against HIV-1, T pallidum, H ducreyi, and HSV-2 infections were tested using approximately 1000 paired (2 year interval) sera collected from a rural adult (15-54 years) population cohort in south west Uganda. RESULTS: Overall HIV-1 prevalence was 4.9%. Prevalence for T pallidum was 12.9% among males and 12.6% among females. The corresponding rates for H ducreyi were 9.8% and 7.3% respectively. HSV-2 prevalence rates were considerably lower in males (36.0%) than in females (71.5%), p < 0.001. Incidence rates for T pallidum per 1000 person years of observation were 8.4 for males and 12.3 for females. The corresponding rates for H ducreyi were 24.6 and 20.0 and for HSV-2 were 73.2 and 122.9 per 1000 person years of observation, respectively. The RR of HSV-2 incidence was 3.69 in HIV seropositive cases versus HIV seronegative after adjusting for age and sex. The corresponding RR for H ducreyi was 3.50 among female HIV positive cases versus negatives with no effect seen in males. Association between HIV-1 prevalence and prevalence of other STIs was significant (Mantel-Haenszel test) for H ducreyi (p = 0.01) and for HSV-2 (p = 0.004) but not for T pallidum (p > 0.4). HSV-2 prevalence was associated with number of lifetime sexual partners (females, p = 0.003; males, p = 0.08). CONCLUSIONS: The results have provided a reliable estimate of the magnitude of the STI problem and demonstrated an association between HIV-1 status and serology of other STIs in a general rural population in sub-Saharan Africa. The study has also highlighted a correlation between HSV-2 seropositivity and number of reported lifetime sexual partners. The present study described the susceptibility of C4D guinea pigs to cutaneous infection with Treponema pallidum subsp. pertenue Haiti B strain. The general manifestations of the disease in adults and neonates differ, to a certain degree, from those induced by T. pallidum subsp. pallidum Nichols strain. Noticeable differences between the infections were reflected in the character of the skin lesions, their onset and persistence, and the kinetics of the humoral response. The incidence and dissemination of cutaneous yaws lesions in very young guinea pigs were remarkably different from the low frequency observed in a similar age group of syphilis infection, 100 versus 17%, respectively. Moreover, as opposed to T. pallidum subsp. pallidum, T. pallidum subsp. pertenue does not cross the placenta. Offspring born to yaws-infected mothers did not produce immunoglobulin M antibodies and their organs, examined by PCR and rabbit infectivity test (RIT), were all negative. Examination of a large number of tissues and organs in adult, neonate, and maternal yaws by PCR and RIT clearly demonstrated that, unlike syphilis, there was a low incidence and short persistence of the yaws pathogen in internal organs. These findings stress the dermotropic rather than the organotropic character of yaws and provide further evidence of distinctive biological and pathological differences between yaws and venereal syphilis. In a survey of 333 pygmies in the Cameroon 4.5 percent. were found to have clinical evidence of yaws. This was also found in 10 per cent. of the inhabitants of camps in Zaire. The results of VDRL and FTA-ABS tests on a sample of the population are presented. These provide evidence of a very high incidence of treponemal disease, 80 to 90 percent. of the population showing serological evidence of infection. Yaws is endemic in rural Guyana. An observational study was conducted to determine the efficacy of oral penicillin V therapy in treating skin lesions of yaws in children. In 1999, inhabitants of 7 rural villages near Bartica, Guyana, were screened for skin lesions of yaws. Cases were confirmed by serological testing. A control program was implemented in 2000: children < or =14 years old were screened, and those with active lesions were treated with oral penicillin V for 7-10 days. In 2001, children were rescreened and active cases were treated. Prevalence of yaws skin lesions fell from 5.1% (52 of 1020 children screened in 2000) to 1.6% (8 of 516 in 2001), a 71% drop. Sixteen (94%) of 17 children treated in 2000 and reassessed in 2001 had complete resolution of lesions. A targeted, oral penicillin-based treatment regimen can successfully treat dermatologic yaws in individual children and can decrease the prevalence of skin yaws in a community in which it is endemic. Yaws is no longer a national and international health priority for intervention, but there is still a negative perception of the disease in the few affected communities. A survey in 1988 in the northern region of Ecuador documented a prevalence of 16.5% of clinical cases and 96.3% of serological cases. A continuous, long-term community-based surveillance programme was therefore put in place focusing on yaws as one of the sentinel diseases. The results of this intervention are reported here. In 1993, a second survey showed a reduction in the prevalence of clinical cases to 1.4% and of serological cases to 4.7%. Between 1993 and 1998, no other clinical cases were detected and the serological prevalence in 1998 was 3.5%, corresponding with clinical cases of primary or congenital syphilis, latent yaws under follow-up, and individuals with low serological titres indicating a 'serological scar'. These data indicate that yaws has been eliminated. Another important outcome of the intervention is the increased self-confidence in the communities that health problems can be tackled. This report presents the results of serological analyses made by the laboratory of the Treponematoses Control Project, Indonesia, from its establishment in April 1951 until April 1953. All sera were tested quantitatively with the VDRL and Kline slide-tests or the Kahn test, or with all three.A study of the mean reagin titre in untreated yaws cases showed that the percentage of seronegative reactors among clinically positive cases was low. Less seronegativity was observed among females than males.EXAMINATION OF DECREASE IN MEAN REAGIN TITRE AFTER TREATMENT BY CLINICAL GROUP SHOWED MAXIMUM TO MINIMUM DECREASE IN THE FOLLOWING SEQUENCE: early contagious, early contagious plus hyperkeratosis, ulcerative plus osteo-articular, ulcerative, hyperkeratosis, and osteo-articular lesions. The decrease tended to be greater in females than males and in patients with high than with low titre; it also varied with the age of the patient. No significant variation in decrease was noted when four different PAM treatment schedules were tested comparatively. The percentage of serological cure and improvement with all schedules was highest in the cases with early lesions, and in the younger age-groups.A study of patients requiring re-treatment at the time of resurvey showed no important difference in mean reagin titre between clinically cured and uncured patients suffering from palmar or plantar hyperkeratosis and ulcerative or osteo-articular lesions.Serological testing of sera from clinically negative household contacts and non-contacts, with or without previous history of yaws, gave the following results: Among the household contacts, the number of seronegative reactors, while not affected by age-distribution, was significantly higher in the history-positive than in the history-negative groups. The percentage of seropositive reactors was in direct proportion to the prevalence of yaws, the seropositivity-rate being high in villages with a yaws incidence of 11%-30%.The report also contains suggestions for improving the conduct of the anti-yaws campaign. The serological aspects of anti-yaws campaigns are discussed. Serological reactions in untreated yaws are considered according to each type of lesion and by age-groups; the serological pattern in types I and II show much similarity in that the majority of cases fall in the high titre levels while with types IV and V there appears to be a more even distribution of cases at all titre levels. A uniform serological response is seen in the early lesions for all age-groups and this is also true of hyperkeratosis cases. There are more type IV cases among the younger age-groups showing high titres, while in the older groups there is an upward trend towards the negative side. Type V also shows a regressive serology.The serological response to treatment with penicillin in three different schedules (4 ml x 2, 2 ml x 2, and a single injection of 4 ml) is then compared. There was no marked difference between the response at the end of one year with all three schedules; with a larger amount of material, however, it is likely that the 2 ml x 2 dosage would be seen to be less effective than the other two schedules. Serological response to treatment with any schedule would be governed by the number of cases having a high initial titre among the group under consideration. The decrease in titre, after being comparatively rapid in the first six months after treatment, is slow, and reversal to seronegativity at the end of a year occurs only in a small number of cases.The role of mass serological examinations in large-scale campaigns is considered. While the serological approach is shown to be the only scientific one, the impracticability of mass serological investigation is brought out. It is stressed, however, that in a mass campaign a serological service is essential for pilot studies, orientation of incidence of latent yaws, check on diagnosis, and assessment of dosage and of the necessity for re-treatment.The possible reasons for prozone phenomena in slide flocculation tests are discussed. The presence of reagin-like substances of extraneous origin to treponemal infection, as well as of reagin, in yaws patients may be responsible for some of these phenomena.
