Musculoskeletal conditions in children attending two Togolese hospitals. (1/19)

OBJECTIVE: A retrospective study was conducted in order to point out the different kinds of musculoskeletal conditions observed in children attending two Togolese hospitals. RESULTS: A total of 434 (242 females, 192 males) of the 29 620 children examined (1.5%) were suffering from these conditions. Probable joint and bone infections (187 patients, 43%), limb deformities (106 patients, 24%), osteochondrosis (60 patients, 14%) and vaso-occlusive crisis due to haemoglobinopathies (29 patients, 7%) were the main conditions observed. Osteomyelitis observed in 128 patients affected the humerus (25 patients), radius (10 patients), femur (68 patients), tibia (15 patients), fibula (five patients), and both tibia and fibula (five patients). Probably, infectious arthritis seen in 30 patients affected mainly the hip (11 patients) and the knee (13 patients). In the spine, infection affected the midthoracic and upper lumbar areas. Underdevelopment, sickle cell anaemia and sickle cell haemoglobin C disease were the main risk factors in determining susceptibility to infections. Vaso-occlusive crises were due to sickle cell anaemia (11 patients) and sickle cell haemoglobin C disease (18 patients). Osteochondrosis seen in 60 patients free from haemoglobinopathy involved the spine (Scheuermann's disease, 38 patients) and the hip (Legg-Calve-Perthes disease, 22 patients). Limb deformities were observed in the knee (varus and valgus deformities in 64 patients) and the foot (talipes varus equin in 40 patients). CONCLUSION: This study's findings, which require further confirmation, suggest some conclusions. Scheuermann's disease can explain in part the degenerative disc conditions observed in African adults. Valgus and varus deformities play an important role in the development of knee osteoarthritis in Black Africa. An African child with joint or bone pain should be investigated for sickle cell anaemia. In the future, improved lifestyle and better health care will be essential to reduce bone and joint infections, and allow refined diagnosis of connective tissue diseases now probably underestimated in African children.  (+info)

Surgical correction of Scheuermann's kyphosis. (2/19)

This is a retrospective study of eight consecutive patients of mean age 19 (13-27) years with severe Scheuermann's kyphosis who underwent anterior and/or posterior fusion using the Cotrel-Dubousset (CD) instrumentation. In two an anterior release and fusion with rib grafts had been previously performed. The mean follow-up was 5 years. The preoperative hyperkyphosis averaged 86 degrees (71 degrees - 99 degrees), which was postoperatively 44 degrees (32 degrees - 58 degrees). The average loss of correction was 4.6 degrees (1 degrees - 12 degrees). The lumbar hyperlordosis spontaneously improved from -67 degrees to -48 degrees. Two patients, who had chronic back pain refractory to conservative treatment, improved considerably after surgery.  (+info)

Scheuermann's disease as a model displaying the mechanism of venous obstruction in thoracic outlet syndrome and migraine patients: MRI and MRA. (3/19)

Kyphosis of the thoracic spine rotates the scapulae anterior laterally, clavicles and subclavius muscles anteriorly, displaces the manubrium posteriorly, which increases the slope of the first ribs. This increases tension on the anterior scalene muscles and the neurovascular bundles which causes brachial plexopathy (TOS). Scheuermann's disease (spinal osteochondrosis; juvenile kyphoscoliosis) is a disorder which consists of vertebral wedging endplate irregularity and narrowing of the intervertebral disk space causing kyphosis of the thoracic spine and may also involve the lumbar space. It occurs at puberty and involves both male and females. Abduction external rotation of the upper extremities (arms overhead) posterior inferiorly rotate the clavicles and the subclavius muscles which enhances tension on the venous drainage and neurovascular supply that diminishes venous return. This triggers complaints of thoracic outlet syndrome (TOS) and migraine headache. Bilateral magnetic resonance imaging (MRI) demonstrates compressing abnormalities of the brachial plexus. Five patients with Scheuermann's disease were imaged with the 1.5 Tesla magnet (Signa; General Electric Medical Systems, Milwaukee, WI) 3-D reconstruction MRI. T1W and T2W pulse sequences were performed in transverse, the coronal, transverse oblique, sagittal, and coronal abduction external rotation planes using 4 mm slice thickness and 512 x 256 matrix size. Water bags were used to enhance the signal to noise ratio. Magnetic resonance angiography (MRA) 2-D Time Of Flight (TOF) was obtained to compression for anatomic display evaluate perfusion of the brachial plexus. MRI and MRA captured sites of brachial plexus. One patient was selected for this presentation, which demonstrates the compression of the brachial plexus and venous obstruction which triggered complaints of thoracic outlet syndrome.  (+info)

Spinal osteochondrosis, previously known as Scheuermann's disease and spinal osteochondritis, is common and is probably becoming increasingly prevalent. Its incidence in patients whose primary complaint was of backache was shown to be twice as great as that in the general population. There was also a significantly higher incidence of lower lumbar spondylosis in patients with previous osteochondrosis compared with those without. Familial Scheuermann disease: a genetic and linkage study. (5/19)

Scheuermann juvenile kyphosis or Scheuermann disease is the most frequent cause of kyphosis in adolescence. However, the natural history and genetic basis is still unknown. Reports of identical radiological changes in monozygotic twins, sib recurrence, and transmission over three generations suggest underlying heritability. In this study, 12 probands were referred to us. Upon radiological examination of the proband's parents and sibs, seven were shown to have familial Scheuermann disease with an autosomal dominant pattern of inheritance. Of the remaining five probands, four had chromosomal anomalies. The three largest pedigrees were subjected to linkage analysis with three candidate genes: Duffy, COL1A1, and COL1A2. Linkage of Scheuermann disease was excluded with Duffy (lod score = -2.195 at theta = 0.10) and COL1A2 (lod score = -2.750 at theta = 0.05) in these families.  (+info)

Thoracic cord compression caused by disk herniation in Scheuermann's disease: a case report and review of the literature. (6/19)

We present the case of a 14-year-old male with Scheuermann's disease and significant neurological deficit due to thoracic disk herniation at the apex of kyphosis. He was treated with an anterior decompression, anterior and posterior fusion in the same setting using plate, cage and a segmental instrumentation system. Scheuermann's Disease, also known as Scheuermann's Kyphosis, is a medical condition that affects the spine. It is a developmental disorder of the vertebral bodies involving anterior wedging of at least three adjacent vertebrae, leading to a progressive rounded or hunchback-like curvature of the upper (thoracic) spine. This deformity can result in a rigid, angular kyphosis and may cause back pain, breathing difficulties, or cosmetic concerns. The exact cause of Scheuermann's Disease is unknown, but it tends to run in families and is more common in males than females. Treatment typically includes physical therapy, bracing, and, in severe cases, surgery.

Osteosarcoma is defined as a type of cancerous tumor that arises from the cells that form bones (osteoblasts). It's the most common primary bone cancer, and it typically develops in the long bones of the body, such as the arms or legs, near the growth plates. Osteosarcoma can metastasize (spread) to other parts of the body, including the lungs, making it a highly malignant form of cancer. Symptoms may include bone pain, swelling, and fractures. Treatment usually involves a combination of surgery, chemotherapy, and/or radiation therapy.

"Bone" is the hard, dense connective tissue that makes up the skeleton of vertebrate animals. It provides support and protection for the body's internal organs, and serves as a attachment site for muscles, tendons, and ligaments. Bone is composed of cells called osteoblasts and osteoclasts, which are responsible for bone formation and resorption, respectively, and an extracellular matrix made up of collagen fibers and mineral crystals.

Bones can be classified into two main types: compact bone and spongy bone. Compact bone is dense and hard, and makes up the outer layer of all bones and the shafts of long bones. Spongy bone is less dense and contains large spaces, and makes up the ends of long bones and the interior of flat and irregular bones.

The human body has 206 bones in total. They can be further classified into five categories based on their shape: long bones, short bones, flat bones, irregular bones, and sesamoid bones.

Osteoporosis is a systemic skeletal disease characterized by low bone mass, deterioration of bone tissue, and disruption of bone architecture, leading to increased risk of fractures, particularly in the spine, wrist, and hip. It mainly affects older people, especially postmenopausal women, due to hormonal changes that reduce bone density. Osteoporosis can also be caused by certain medications, medical conditions, or lifestyle factors such as smoking, alcohol abuse, and a lack of calcium and vitamin D in the diet. The diagnosis is often made using bone mineral density testing, and treatment may include medication to slow bone loss, promote bone formation, and prevent fractures.

Bone neoplasms are abnormal growths or tumors that develop in the bone. They can be benign (non-cancerous) or malignant (cancerous). Benign bone neoplasms do not spread to other parts of the body and are rarely a threat to life, although they may cause problems if they grow large enough to press on surrounding tissues or cause fractures. Malignant bone neoplasms, on the other hand, can invade and destroy nearby tissue and may spread (metastasize) to other parts of the body.

There are many different types of bone neoplasms, including:

1. Osteochondroma - a benign tumor that develops from cartilage and bone
2. Enchondroma - a benign tumor that forms in the cartilage that lines the inside of the bones
3. Chondrosarcoma - a malignant tumor that develops from cartilage
4. Osteosarcoma - a malignant tumor that develops from bone cells
5. Ewing sarcoma - a malignant tumor that develops in the bones or soft tissues around the bones
6. Giant cell tumor of bone - a benign or occasionally malignant tumor that develops from bone tissue
7. Fibrosarcoma - a malignant tumor that develops from fibrous tissue in the bone

The symptoms of bone neoplasms vary depending on the type, size, and location of the tumor. They may include pain, swelling, stiffness, fractures, or limited mobility. Treatment options depend on the type and stage of the tumor but may include surgery, radiation therapy, chemotherapy, or a combination of these treatments.

Bone remodeling is the normal and continuous process by which bone tissue is removed from the skeleton (a process called resorption) and new bone tissue is formed (a process called formation). This ongoing cycle allows bones to repair microdamage, adjust their size and shape in response to mechanical stress, and maintain mineral homeostasis. The cells responsible for bone resorption are osteoclasts, while the cells responsible for bone formation are osteoblasts. These two cell types work together to maintain the structural integrity and health of bones throughout an individual's life.

During bone remodeling, the process can be divided into several stages:

1. Activation: The initiation of bone remodeling is triggered by various factors such as microdamage, hormonal changes, or mechanical stress. This leads to the recruitment and activation of osteoclast precursor cells.
2. Resorption: Osteoclasts attach to the bone surface and create a sealed compartment called a resorption lacuna. They then secrete acid and enzymes that dissolve and digest the mineralized matrix, creating pits or cavities on the bone surface. This process helps remove old or damaged bone tissue and releases calcium and phosphate ions into the bloodstream.
3. Reversal: After resorption is complete, the osteoclasts undergo apoptosis (programmed cell death), and mononuclear cells called reversal cells appear on the resorbed surface. These cells prepare the bone surface for the next stage by cleaning up debris and releasing signals that attract osteoblast precursors.
4. Formation: Osteoblasts, derived from mesenchymal stem cells, migrate to the resorbed surface and begin producing a new organic matrix called osteoid. As the osteoid mineralizes, it forms a hard, calcified structure that gradually replaces the resorbed bone tissue. The osteoblasts may become embedded within this newly formed bone as they differentiate into osteocytes, which are mature bone cells responsible for maintaining bone homeostasis and responding to mechanical stress.
5. Mineralization: Over time, the newly formed bone continues to mineralize, becoming stronger and more dense. This process helps maintain the structural integrity of the skeleton and ensures adequate calcium storage.

Throughout this continuous cycle of bone remodeling, hormones, growth factors, and mechanical stress play crucial roles in regulating the balance between resorption and formation. Disruptions to this delicate equilibrium can lead to various bone diseases, such as osteoporosis, where excessive resorption results in weakened bones and increased fracture risk.

Bone density refers to the amount of bone mineral content (usually measured in grams) in a given volume of bone (usually measured in cubic centimeters). It is often used as an indicator of bone strength and fracture risk. Bone density is typically measured using dual-energy X-ray absorptiometry (DXA) scans, which provide a T-score that compares the patient's bone density to that of a young adult reference population. A T-score of -1 or above is considered normal, while a T-score between -1 and -2.5 indicates osteopenia (low bone mass), and a T-score below -2.5 indicates osteoporosis (porous bones). Regular exercise, adequate calcium and vitamin D intake, and medication (if necessary) can help maintain or improve bone density and prevent fractures.

Joints can be classified in several ways, including structure, function, and the type of tissue that forms them. The three main types of joints based on structure are fibrous (or fixed), cartilaginous, and synovial (or diarthrosis). Fibrous joints do not have a cavity and have limited movement, while cartilaginous joints allow for some movement and are connected by cartilage. Synovial joints, the most common and most movable type, have a space between the articular surfaces containing synovial fluid, which reduces friction and wear.
